Requirements for Registration When registering with Sonographers Medical you will be required to upload or post the following documentation to us: An up-to-date CV Evidence of Immunities. (click here for full details of immunities) Completed Health Declaration. Proof of Identification . Eg Passport. (Originals will be inspected at interview). Proof of Right to Work in the UK, where relevant. Proof of residential address. UK State Registration certificate, where relevant. Eg. HPC. GMC. etc Qualification certificates. (Originals will be inspected at interview). Certificates of CPR, Manual Handling and any other training with in the past 12 months Any CPD certificates dated with in the past three years. A colour passport style Photograph.

In addition, we will post you a CRB application which will need to be mailed back to us (electronic and fax copies are not permitted). 

We are unable to offer you any work until the CRB Disclosure has been issued to us by the Criminal Record Bureau.
